How do I get back to my home world where I came from, if I only have memories and a connection to my friend?

Please don't call me crazy, don't tell me it's time to see a doctor or anything like that, also please don't write jokes, it hurts me a lot.

How can I return to my world, both consciously and physically, if I don't know how, I have only memories of my world and my friend, and I also have a connection with my friend and we can sometimes communicate.

In my world I lived in a house in the forest, not so far from a village, but at the moment it is unfortunately abandoned, and there are only empty houses and furniture and things that were not taken away in a hurry.

My friend used to be a doctor, before he was cursed, and he didn't have to live in a place far away from people, he found me in the forest, after our village was destroyed, or rather burned down, because they suspected some of its inhabitants in the curse, but I miraculously survived, and I was taken by my friend, and we started to live in that cabin.

My friend also doesn't know how I can get back to my world, to him, and except for death, or some other way - I have no idea how to get back to my world, and I don't know what to do.

At the moment I am 16, in this world, and in my world I am less.

Yes, in my world everything is older, there are no cars, no internet, no light bulbs (at least I've never seen them in my life).
But book printing is more developed there, and it's available even in a not so big city.
The houses are mostly one to two stories.
There is a king, but all I know about him is that he issued a decree and people had to move outside the walls.

Because there is no simple, or easy way "out", I'm afraid. Death is not a simple or easy way out either. Half the time (or most of the time in some cases), you only end up dropping even "lower", and then it gets even harder (orders of magnitude so) to "get out" of those more exotic places. And if you don't like being here, you definitely won't enjoy being there.
However, this place has it's perks actually. For one, you can be in more than one place at the same time. With only a little bit of effort, commitment and patience, one can get out without actually leaving. It's quite neat. You've got lots of time on your hands, might as well give that a try.
